Quick Reference
Ventilation Modes
  • Volume Control (VC/A-CMV)

Set tidal volume with constant flow. Pressure varies based on resistance/compliance.

  • Pressure Control (PC/P-CMV)

Fixed inspiratory pressure with decelerating flow. Volume varies with lung mechanics.

  • Pressure Support (PS/PSV)

    Patient-triggered, pressure-limited, flow-cycled breaths. Used during weaning.

  • SIMV + PS

Combines mandatory breaths (VC/PC) with spontaneous PS breaths between.

Essential Parameters
  • Tidal Volume (Vt)

    4-8 mL/kg IBW. Lung-protective: 4-6 mL/kg for ARDS.

Monitor for plateau pressure <30 cmH₂O

  • Respiratory Rate (RR)

    12-25 breaths/min. Adjust for pH/PaCO₂ goals.

May need higher rates with low Vt strategy

  • PEEP

5-24 cmH₂O. Titrate based on FiO₂ requirements.

ARDSnet protocol: PEEP/FiO₂ tables for severe hypoxemia

  • FiO₂

Target SpO₂ 88-95%. Minimize when possible.

Lower SpO₂ targets may be appropriate in COPD

Troubleshooting
  • Low SpO₂

↑ FiO₂, ↑ PEEP, consider recruitment, check for disconnects/leaks

  • High PaCO₂

↑ RR, ↑ Vt (if safe), check for air trapping/auto-PEEP

  • High Airway Pressure

Check for obstruction/secretions, verify tube position, consider bronchodilators

  • Patient-Ventilator Dyssynchrony

Adjust trigger sensitivity, inspiratory time, flow pattern, consider sedation

  • Auto-PEEP

↓ RR, ↑ expiratory time, bronchodilators, consider external PEEP
